Please note that some forms contain instruction relating to their lodgement which may not always be current, expecially where they form part of a publication that is periodically contract printed such as score books. Any such instruction offered on forms is for guidance only and should be checked against the relevant regulations.
Any reference to the 'Association Secretary' or other association official is a reference to the person holding that position with the BHRDCA, not the umpires' association which is an affiliate of the BHRDCA.

ADMINISTRATION
Affiliation - Teams'Registration & Applicationto become an Affiliate member or Associate of the BHRDCA
Officials Registration - Affiliates/Associates Registration of Officials, contact details etc
Clearance/Permit Application To be used for clearance applications from BHRDCA affiliated clubs or permit applications for players requiring them under rule 10(c)(iv) and (d)(ii) to play with a BHRDCA affiliated club having already played with a club in another association during the same season. Also for permit applications under rule 10(d)(vii).
Permit Application To be used for permit applications when seeking to play someone who does not otherwise qualify to play in the desired grade under rule 10(d)(iii) and (v).
Match Report - redundant
Umpires' Report on Match To be completed by umpire(s) after each game and lodged with the Secretary no later than 8pm on the ensuing Tuesday or as directed by the Committee of Management. Umpires must advise "Player Reports" both verbally and by forwarding a copy of the quadruplicate form to the Association Secretary (John Toogood) not more than 24 hours and 48 hours respectively after completion of the match. This requirement is in addition to lodgement of the attached umpires' general report on the match upon which any "Code of Behaviour Violation" must be advised.
Captain's Report on Umpire(s) Also in the back of BHRDCA scorebooks this form is to be filled in (if applicable) by the home team captain (or junior team manager) having consulted with his visting counterpart following each game. Reports must be lodged by the club of the home team with the Registrar (Ron Hennessey) no later than 8pm on the ensuing Tuesday or otherwise as directed by the Committee of Management. Please note that this form differs slightly from the scorebook version in that it provides for both umpires to be dealt with on a single form. Lodgements emailed as an attachment or transmitted by facs' will be accepted.
Claim Forms - Player Averages All clubs are required to lodge a player averages claim form by the due date whether or not they have any qualifiers, in which case the lodgement of a form with the club's name, relevant grades and "nil qualifiers" noted is necessary.
2-Day Competition - use this form for both open and under-18 competitions
Umpires' Report on Match To be completed by umpire(s) after each game and lodged with the Secretary no later than 8pm on the ensuing Tuesday or as directed by the Committee of Management.
Manager's Report on Umpire(s) Also in the back of BHRDCA scorebooks this form is to be filled in (if applicable) by the home team manager having consulted with his visting counterpart following each game. * Because we comply with the VMCU standard draw but don't use all available dates of play the posted round numbers for 1-day and under-age competitions are out of sinc. As a result you should rely only on fixture dates and ignore the round numbers.
